Patricia Nguyen, MSW
With over 18 years of experience in geriatric social work across Houston's largest hospital systems, Patricia founded Hawthorne Senior Care Consultants after witnessing firsthand how overwhelming the care transition process can be for families. She has personally guided more than 400 Greater Houston families through care assessments, facility placements, and long-term planning. Patricia holds a Master of Social Work from the University of Houston and is a licensed clinical social worker with specialized certifications in geriatric care management.
